Appointment of Medical Treatment Decision Maker & Enduring Power of Attorney (Financial and Personal)

Using this interview you can create one or both of the following documents:

Appointment of Medical Treatment Decision Maker: this document enables you to appoint a person to make medical treatment decisions for you in the event that you are unable to make these decisions for yourself; and/or

Enduring Power of Attorney (Financial and Personal): this document enables you to appoint a person/s to make financial and personal decisions for you. Financial decisions include things such as paying bills and making financial transactions on your behalf. Personal decisions include matters such as where you live and who you spend time with. This document enables your attorney to make decisions for you now (at your direction) and also when you are unable to make those decisions yourself.


Before getting started:

You will need to know whether you want to create just one of the documents or both, and who you would like to appoint as your medical treatment decision maker(s) and/or attorney(s).

Please note that:

1.   You are only able to appoint a single Medical Treatment Decisions Maker with an alternative person appointed as your second medical treatment decision maker/third medical treatment decision maker etc.; and

2.   You are able to appoint two or more people to act jointly and/or severally as your attorney in relation to financial and personal matters.


Interview time:

This interview should take you 5 to 10 minutes. You can save your responses and return to complete the interview at any time.


Document length:

Approximately 5 pages for the Enduring Power of Attorney; and

Approximately 6 pages for the Appointment of Medical Treatment Decision Maker.
