You can use this Power of Attorney to appoint one or more attorneys to make decisions and act on your behalf. Please note, the Power of Attorney is automatically revoked if you lose mental capacity. If you want to make a Power of Attorney that continues after you have lost mental capacity then you need to make an Enduring Power of Attorney.
